Party Request Form
Thank you for choosing us as your party host. We are certain your child and their friends will have a great time. It is important that you know exactly what we provide, and what you are going to provide...We wouldn't want to miss a thing on this special day!
WE WILL PROVIDE:
• A 20,000 square foot facility with state of the art equipment for any of the activities you may want.
• One instructor for up to 13 children..an additional instructor will be provided for 14 - 24 children.
• Table cloths, plates and plastic utensils.
• Fun and Games for 45 minutes, celebration for 45 minutes...we clean up.
Our parties last 90 minutes...45 minutes in the gym and 45 minutes to have cake, ice cream, etc. If you would like the portions to be different, please let us know.
When filling out the form below, please help us with our party plan and indicate any preferences you and your child may have (age appropriate, please). You may choose several different activities if you wish. Sorry...no "big" trampoline during birthday parties.
